Honda is recalling more than 259,000 vehicles in the U.S. due to a brake pedal defect that could hinder a driver’s ability to stop or slow down, increasing the risk of crashes or injuries.

The recall includes certain 2021-2025 Acura TLX, 2023-2025 Acura MDX, and 2023-2025 Honda Pilot vehicles. According to the National Highway Traffic Safety Administration, the brake pedal pivot pin in these vehicles was not properly secured during production, which may cause the pedal to shift out of position.

“The issue could also cause an abnormal brake pedal feeling during operation, illuminate the brake malfunction light in the instrument cluster, or cause the vehicle’s brake lights to remain lit even when the brake pedal is not applied,” American Honda said in a statement issued on Wednesday.

The defect results in non-compliance with Federal Motor Vehicle Safety Standards 108, covering lamps and reflective devices, and 135, addressing light vehicle brake systems.

Honda estimates that 1% of the 259,033 affected vehicles have this issue. Dealers will inspect and, if necessary, replace the brake pedal assembly free of charge. Owner notification letters are expected to be mailed on July 28, 2025. Dealer notifications began June 13, 2025.

Honda began investigating the issue in April 2024 after a report of a vehicle with a faulty brake pedal. The company traced the problem to improper assembly at a U.S. supplier plant, which has since closed. Production moved to a Mexican facility, where a camera sensor now verifies proper pivot pin installation.

Between February 2024 and June 5, 2025, Honda received three warranty claims related to the issue, but no reports of injuries. The manufacturer’s report was filed with NHTSA on June 12, 2025, under campaign number 25V391.
