Fort Worth will expand its High ImpACT homelessness program with $2.18 million annually for four years, targeting the city’s most vulnerable residents.

The initiative focuses on individuals with severe mental illness who frequently encounter law enforcement and struggle to find stable housing. The decision comes after a pilot program demonstrated remarkable success rates in one of the nation’s most challenging housing markets.

The program achieved a 98% housing retention rate during its pilot phase, according to city data. Additionally, 89% of those offered housing accepted placements within six months, exceeding the city’s original targets.

“This is not an insignificant price tag, but the work that we’re able to do through this program has really made a tremendous amount of difference,” Council Member Elizabeth Beck said.

The High ImpACT initiative deploys specialized teams across seven high-impact areas citywide. These teams include physician assistants, mental health professionals, and landlord engagement coordinators who work together to address complex cases.

Partnership Home collaborates with My Health My Resources of Tarrant County and Acclaim Health to provide comprehensive support services. The program utilizes an Assertive Community Treatment model for both mental and physical healthcare needs.

Fort Worth Housing Solutions recently launched Casa de Los Sueños, the city’s first development integrating comprehensive services for homeless families with children. The project provides permanent, affordable housing alongside professional, educational, and health-related support services.

Tarrant County coordinates these efforts through partnerships with Fort Worth, Arlington, and the Continuum of Care organization. Monthly and quarterly meetings ensure effective allocation and evaluation of homelessness services across the region.

The county’s 2025 Consolidated Plan emphasizes transitional housing as a pathway to permanent placement. This approach combines temporary housing with supportive services designed to provide stability during the transition process.
